We would like to thank you for taking part in Trinity Christian School's new process of submitting athletic participation paperwork online. Please review all instructions before completing the electronic forms.

• To access the forms, click on “Proceed to Online Forms” at the bottom of the page. If you already have a parent account, log into your account to view and complete the forms. If you do not have a parent account, you can create an account before completing your student's forms or complete the forms using the “Continue as a guest” option. When creating an account or continuing as a guest, you will need to know your child's Student ID number. Your child's Student ID number is located on your Parent Blackbaud account under your child's "Contact Card". The parent account will allow you to review your student’s compliance status and access your paperwork as needed. You may also log into your parent account by downloading the Rank One mobile app .*

How to Create a Parent Account Guide

• Please read each form carefully and place a check in each box to signify that you and your student understand and accept each form.

• You will need to upload your child's physical under "Physical Upload Form. You will upload 4 pages individually (History Form page 1, History Form page 2, Physical Examination Form, and Medical Eligibility Form).

• To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box.

• Once you have completed both sections (Physical Upload Form and Participation Signature Page), the Athletic Department will approve or decline the forms. You will receive an email confirmation once the forms are completed and approved/declined
